Formula Explained

The Friedewald equation estimates LDL indirectly from the standard lipid panel. In mg/dL units: VLDL ≈ triglycerides ÷ 5, then LDL = total − HDL − VLDL. In mmol/L units, the VLDL divisor changes to 2.2 because of molecular-weight conversion between cholesterol and triglycerides. This is the method many labs use when a direct LDL measurement is not ordered.

Example: total 200 mg/dL, HDL 50 mg/dL, triglycerides 150 mg/dL → VLDL = 150 ÷ 5 = 30 mg/dL → LDL = 200 − 50 − 30 = 120 mg/dL.

The equation assumes a standard ratio of triglycerides to VLDL; in people with very high or very low triglyceride levels, this assumption can distort the LDL estimate.

Worked Examples

Borderline-high panel (mg/dL): total 220, HDL 45, triglycerides 150. VLDL = 30, so LDL = 145 mg/dL — borderline high.
